Services We Provide
Continuing Life With Respect
Our vast array of resources offer choices many families need. As we coordinate care, we’re able to assure all the members on our team are providing top quality care. This includes conversations between physicians and our medical director to make sure referrals are handled smoothly and medications are provided in a timely manner. Clergy members and social workers are included in conversations to keep them abreast of needs, whether that means a home-cooked meal for a family or a mood-lifting bouquet of flowers.
Our primary services include:
- Physician services for the medical direction of the patient’s care, provided by the patient’s personal physician and the hospice medical director
- Home care visits by registered nurses and licensed practical nurses to assess and monitor the patient’s condition
- Pain and symptom management
- Assistance and education to enable the family to provide care at home
- Chaplain services for the patients and/or loved ones, if desired
- Home health aide and homemaker services that address the patient’s personal needs
- Bereavement counseling to help patients and their families deal with grief and loss
- Psychosocial assessment, counseling services, resource identification, and referrals
- Medications for pain and symptom management related to the terminal illness
- Medical supplies and medical equipment
- Physical, speech, and occupational therapy, when appropriate
- Dietary counseling, when appropriate
- Volunteer support to assist the patient and loved ones
