Qualification Criteria

Medicare’s disease specific admission criteria determine eligibility. The following symptoms may be signs that an individual may qualify for hospice services:

• Increased assistance needed with ADL’s
• Dysphasia with recurrent aspiration
• General cognitive decline
• Failure to recover from recent illness
• Unexplained weight loss
• Decreased response to treatments
• Overall negative change in condition
• Increased hospitalizations or emergency room visits
