The Enrollment Process

It is sometimes difficult to know when it is the right time to seek out hospice care. Patients and family members should discuss hospice care as an option, whenever someone is facing a life-threatening illness and curative care is no longer an effective option.

We understand that the decision to stop curative care can be stressful and difficult, and having qualified staff available to discuss any concerns with the patient, family and physician.

Our admissions team consists of a registered nurse, an admission coordinator and social worker. Anyone can make a referral for hospice care, including a patient, family member or physician.

Once a referral is made, our admissions team will contact the enrollee and/or family, and arrange for an appointment, in order to assess the enrollee's condition and discuss hospice services and benefits. The designated physician is contacted to verify the eligibility and confirm the decisions made. Enrollees are asked to sign a consent form confirming the understanding that the care provided is palliative in nature.
